Individualized, youth-focused planning for a permanent family that involves the youth's voluntary and active participation.

Assistance for youth seeking to identify connections or to build or mend relationships.

Youth-specific recruitment that reaches out to the community to find permanence for the youth.

Post Adoption Services such as support groups and therapy for the project's youth and foster and adoptive families.

Training for adults who provide permanent connections for the project's youth on how to be a parent to an adolescent.

FAMILY BOUND

The Family Bound Program prepares youth for family life by encouraging them to think about what they expect from a family and explore many aspects of family life. This program helps youth explore permanency, identify important relationships in their past, and grieve the loss of significant former relationships. The youth then "practice" what they learn in an actual family setting. The program also involves support from other youth and social workers.

YOUTH SUPPORT GROUP

Youth Support Group helps youth work on three tasks:

Clarification (reasons for being in foster care), Integration (mapping all the places they've been) and Actualization (the ability to visualize relationships with others).

Through the use of Life Books and a Life Map, youth can better understand the answers to 5 important questions:

Who am I?
What Happened to Me?
Where am I going?
How will I get there?
How will I know I belong?

The Youth Support Group is currently only in Sacramento County.
